About the role
Key responsibilities & impact- Provide personalized, one-on-one skilled nursing care to patients in their homes
- Work under the direction of an RN
- Implement individualized care plans with patients, families and the healthcare team
- Educate patients and families on disease management, treatment options and self-care
- Maintain accurate patient-care records, including visits, interventions and outcomes
- Communicate promptly with patients, physicians, clinical managers and healthcare team members about needs and status changes
- Participate in care coordination and discharge planning
- Support patients' recovery, rehabilitation and independence
Requirements
What you’ll need- Current nursing license in the practicing state
- Valid driver's license, auto insurance and reliable transportation
- Current CPR certification
- One year's experience as an LPN/LVN in a clinical setting
- Nursing experience in a Home Health or Hospice setting preferred
- Ability to provide skilled nursing care in patients' homes under the direction of an RN
- Must maintain personal vehicle liability insurance in accordance with state requirements or specified minimum limits
