About the role

Key responsibilities & impact
  • Provide personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN
  • Implement individualized care plans with patients, families, and the healthcare team
  • Educate patients and families on disease management, treatment options, and self-care techniques
  • Maintain accurate records of patient visits, interventions, and outcomes
  • Communicate promptly with patients, physicians, clinical managers, and healthcare team members about needs and status changes
  • Participate in care coordination activities and discharge planning
  • Report to the Clinical Manager
  • Support patients recovering from injury, illness, surgery, hospitalization, or managing chronic conditions

Requirements

What you’ll need
  • Current nursing license in the practicing state
  • Valid driver's license, auto insurance and reliable transportation
  • Current CPR certification
  • One year experience as an LPN/LVN in a clinical setting with adult patients
  • Nursing experience in a Home Health or Hospice setting (preferred)
  • Must maintain personal vehicle liability insurance in accordance with state minimum limits or specified Humana limits
  • Patient-facing role requiring TB screening if selected
