About the role
Key responsibilities & impact- Provide comprehensive care to seniors following clinical standards
- Collaborate daily with physicians, pharmacists, care coaches, behavioral health specialists, and other care team members
- Manage referrals, hospital/SNF coordination, DME, and home health services
- Document care with support from quality-based coders
- Participate in on-call rotation
- Support growth plans at the center
- Report to the Area Medical Director
- Deliver proactive, preventive, personalized senior primary care, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment
Requirements
What you’ll need- Bachelor's degree
- 8 or more years of technical experience
- 2 or more years of project leadership experience
- Active, unrestricted Nurse Practitioner/PA license in the appropriate state
- Required to undergo tuberculosis (TB) screening if selected
- Passion for contributing to an organization focused on improving consumer experiences
- Willingness to participate in limited on-call coverage by phone
- Experience providing value-based care
- Patient-facing role
- Ability to work 40 scheduled hours per week
