About the role

Key responsibilities & impact

Assess and screen patients’ daily living and work-related skills
Develop therapeutic retraining programs with measurable objectives
Administer and interpret diagnostic and prognostic functional tests in patients’ homes or other settings
Confer with physicians and healthcare team members
Participate in development and revision of Plans of Care Treatment
Provide therapeutic treatment and instruction to improve or restore strength, coordination, range of motion, and function
Review and expand retraining programs according to patient behavioral goals and rehabilitation guidelines
Develop and implement vocationally oriented plans with vocational team members
Report patient status and progress to physicians, rehabilitation staff, Clinical Manager, patients, and families
Participate in care coordination and discharge planning
Act as a resource to healthcare personnel
Design community reintegration activities and coordinate plans with the interdisciplinary team
Recommend or design adaptive equipment
Train staff and family members in retraining programs
Coordinate, oversee, supervise, instruct, and evaluate Occupational Therapy Assistants and Home Health Aides
Document patient care observations, interventions, and evaluations on the day services are rendered
Document and implement interim physician orders
Submit evaluations, treatment plans, and discharge summaries to supervisors and care management staff

Requirements

What you’ll need

Degree from an accredited Occupational Therapy Program
A minimum of six months of occupational therapy experience preferred
Home Health experience a plus
Current and unrestricted OT licensure
Current CPR certification
Good organizational and communication skills
A valid driver’s license
Auto insurance
Reliable transportation
Ability to work 40 hours per week
