About the role

Key responsibilities & impact
  • Provide outpatient primary care services to adult and geriatric populations
  • Maintain accurate and up-to-date medical histories and records
  • Manage moderately complex to complex medical conditions
  • Refer patients to specialists
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Contribute to departmental strategy and quality improvement programs
  • Work onsite in outpatient clinics
  • Cover three outpatient clinic locations: CenterWell Redbird in Dallas, DeSoto, and Grand Prairie
  • Provide care during clinic hours Monday–Friday, 8:00 AM–5:00 PM

Requirements

What you’ll need
  • Graduate of an accredited Doctor of Medicine or Doctor of Osteopathic Medicine program
  • Active medical license in the state of jurisdiction
  • Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine
  • Preference for value-based care experience, or at least 2 years of caring for Geriatric patients with high acuities
  • Passion for improving patient experiences and outcomes
  • Participation in Humana/Senior Bridge's Tuberculosis (TB) screening program
  • Specialty in Family Medicine, Internal Medicine, Med-Peds, or Geriatrics
  • Experience working with senior populations, value-based, or primary care models
  • Familiarity with electronic health records (EHR) and digital documentation
  • Availability of at least three days per week
  • Must be willing to travel between assigned clinic locations
