About the role

Key responsibilities & impact
  • Deliver personalized home health physical therapy care focused on functional improvement
  • Administer the physical therapy treatment plan in patients' homes as developed by the Physical Therapist
  • Administer active and passive manual therapeutic exercises, massage, traction, heat, light, cold, water, and electrical modalities
  • Document client records and submit clinical documentation to the office weekly
  • Attend educational meetings and coordinate department in-services
  • Notify the Physical Therapist or Branch Director about equipment failures, supply shortages, or staff/client changes
  • Apply infection control concepts and universal precautions during patient care
  • Cover patients in Montgomery, Cheatham, and Robertson Counties

Requirements

What you’ll need
  • Current state license to practice Physical Therapy
  • Valid driver's license
  • Auto insurance and reliable transportation
  • Current CPR certification
  • Experience collaborating with a team of healthcare professionals
  • Comprehensive knowledge of physical, emotional, social, and biological changes in the geriatric population
  • Read, write, and speak fluent English
  • One year of experience as a Physical Therapy Assistant (preferred)
  • Live in or near Montgomery, Cheatham or Robertson County (preferred)
  • Required TB screening if selected
  • Maintain personal vehicle liability insurance according to state minimum requirements or specified limits
