About the role
Key responsibilities & impact- Provide skilled nursing care to patients in their homes
- Administer medications
- Perform wound care
- Conduct patient assessments
- Develop and implement individualized care plans with physicians and healthcare professionals
- Monitor patient conditions and report changes
- Educate patients and families on disease management, medication, and treatment options
- Maintain accurate patient-care records
- Coordinate with healthcare professionals
- Report patient care and condition progress to physicians and the Clinical Manager
- Oversee Home Health Aides and monitor documentation completion
- Help patients regain independence and recover in their homes
Requirements
What you’ll need- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license
- Auto insurance
- Reliable transportation
- One year nursing experience preferred
- Home Health experience preferred
- Must maintain personal vehicle liability insurance in accordance with residing state minimum limits or specified Humana limits, whichever is higher
- Must complete TB screening if selected
- Scheduled for 40 hours per week
