About the role

Key responsibilities & impact

Support members’ optimal wellness by coordinating services and resources
Identify and resolve barriers that hinder effective care
Monitor patient care using assessments, data and conversations with members
Ensure patients progress toward desired outcomes
Apply professional concepts, standards, regulations, strategies and operating standards
Make decisions regarding work approach and priorities while following direction
Report directly to the Manager, Care Management

Requirements

What you’ll need

2 or more years of related experience
Proficiency with Outlook, Word, Excel, SharePoint and Teams
Ability to use a variety of electronic information applications/software programs
Exceptional verbal/written communication and interpersonal skills
Passion for contributing to an organization focused on continuously improving consumer experiences
Minimum home internet speed of 25 Mbps download and 10 Mbps upload
Dedicated workspace without ongoing interruptions to protect member PHI / HIPAA information
