About the role
Key responsibilities & impact- Lead the company in delivering patient-centered care focused on patient experience, safety, and satisfaction
- Supervise Case Management, Utilization Review, and discharge planning activities
- Facilitate appropriate alternative placements for patients
- Streamline healthcare delivery to promote timely care and effective resource utilization
- Oversee case management processes and identify barriers across the continuum of care
- Coordinate internal DRG review, contracted third-party payers, hospice contracts, and managed-care utilization issues
- Oversee throughput activities and ensure timely patient discharge or escalation
- Monitor staff productivity and performance quality
- Conduct annual evaluations and provide face-to-face feedback
- Collaborate with vendors, outside partners, hospitalists, and physicians on compliance, length of stay, quality metrics, and patient services
- Support the hospital-wide Performance Improvement Program and continuous quality improvement
- Oversee education and consultation programs addressing patients’ biopsychosocial needs
- Help lead efforts to earn and retain Joint Commission certification and other government-program requirements
- Attend meetings, respond to emails promptly, and provide effective leadership when situations require additional effort
Requirements
What you’ll need- Graduate of an accredited RN Nursing Program
- Bachelor’s Degree in Nursing or related field preferred
- Case Management experience in an Acute Care Hospital; 5 years or more required
- Ability to communicate effectively verbally and in writing
- Must be able to work in a union environment
- Three years progressive management experience in the field of expertise preferred
- Case Management certification
- California Registered Nurse (RN) License
- Basic Life Support (BLS) from the American Heart Association (AHA)
- Los Angeles County Fire & Safety Card required within 30 days of hire
- Assault Response Competency (ARC) required within 30 days of hire
