About the role

Key responsibilities & impact

Provide behavioral health services to children, adolescents, parents, adults, and seniors
Provide life skills training, including IADLs, daily functioning, organization, and applications
Assess clients’ and families’ ability to access and use services and supports independently
Deliver case management services, including referrals, linkage, and care coordination
Develop and implement individualized service plans
Collaborate with clients, families, therapists, and multidisciplinary teams to support treatment goals
Assist clients in accessing medical, educational, social, and community resources
Monitor client progress and adjust service plans as needed
Complete timely progress notes, service plans, billing, and other documentation
Maintain compliance with Medicaid and organizational standards
Work under the direct supervision of the Client Support Specialist Team Lead
Serve a caseload of approximately 20–25 clients
Travel within the service area

Requirements

What you’ll need

Bachelor’s degree in social work, Psychology, Counseling, or related field (required)
Minimum 1 year of mental health case management experience working with at risk children, teens, families, adults, and seniors (preferred)
Valid driver’s license and reliable transportation
Willingness to travel locally (up to ~75%)
Proficiency in Microsoft Word, Excel, and Adobe
Ability to complete electronic documentation and billing
Excellent working knowledge of behavioral treatment for children and adolescent behavioral health
Ability to work through assessments of behavioral health conditions
Ability to collaborate and work as a team with therapists and joint staff in the treatment of psycho-social issues
Sensitivity to low income and ethnic minority community
Experience working with Medicaid populations
Fluency in Spanish and English preferred
