About the role

Key responsibilities & impact

Conduct initial and follow-up telehealth assessments on flagged home-health patients
Initiate or adjust plans of care
Coordinate with referring and primary care providers via secure video/audio platform
Interpret trend-based insights from the Circadia platform and prioritize high-risk patients
Drive timely interventions to prevent decline and unnecessary ED visits
Complete documentation and charts within the same day
Complete 25+ tele-visits per day for designated home-health patients in the Southern California/Orange County area
Focus on new admissions, high-acuity flagged patients, and Circadia alerts
Surface insights to improve virtual workflows and technology integration in home health
Support Nurse Practitioner onboarding and share best practices
Partner with clinical operations and product teams
Meet KPIs including addressing flagged patients within 24 hours, completing AI-assisted notes within 24 hours, and improving workflow adherence and patient outcomes

Requirements

What you’ll need

Certified and licensed Nurse Practitioner in California; ANCC (or equivalent) in Adult/Family/Geriatric/Acute
Eligible for Medicare participation; company bills on your behalf
2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently
Comfortable conducting telehealth visits and navigating virtual care platforms
Thrives in a dynamic, evolving startup environment
English required
Tech-savvy: Google Suite, multiple EMRs, Slack, and AI transcription/tools
HCHB experience is a plus
Spanish preferred
