About the role
Key responsibilities & impact- Travel to patient homes across the listed service areas in Orange County and surrounding areas
- Take and record patient vitals during home visits and transmit accurate readings through the Circadia platform
- Install Circadia devices at designated patient locations for the Home Health Program
- Facilitate patients' initial telehealth visits with the Home Health Nurse Practitioner
- Educate patients on device use and telehealth visit setup
- Perform routine device maintenance, IT setup, configuration, and medical equipment assembly
- Coordinate with internal teams throughout the customer lifecycle to support successful technology deployment
- Audit and maintain the asset management list
- Support the Procurement Team with supply chain planning for future installations and company growth
- Come into the office once a week or as needed to pick up supplies
Requirements
What you’ll need- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification required
- Demonstrated experience taking and recording vital signs, including blood pressure, pulse, respiration, temperature, and pulse oximetry
- Skilled with power tools and equipment for mounting and installation
- Valid driver's license and reliable transportation required for daily travel between patient homes
- Comfortable working independently with minimal supervision
- Strong communication and customer service skills, especially with elderly or vulnerable populations
- Familiarity with HIPAA and patient privacy standards
- Willingness to pass a background check
- Mission-driven and passionate about helping patients receive proactive care
- Bilingual Spanish is a plus
