About the role

Key responsibilities & impact
  • Promote rational, safe, and cost-effective drug therapy in collaboration with primary care providers, specialists, nurse care managers, community health partners, and non-clinical roles
  • Identify and resolve medication therapy opportunities while co-designing individualized therapeutic regimens for chronic disease management and preventative care
  • Provide direct care to members and caregivers through Comprehensive Medication Management (CMM)
  • Support hub-based and virtual clinical care teams with pharmacist services
  • Develop medication management action plans with measurable parameters and monitoring timelines
  • Collaborate with Cityblock and external providers to align interventions and encounters with medication-related goals
  • Provide drug information, dosing, interaction, adverse-effect avoidance, and member-specific support
  • Deliver medication-management in-services and develop educational tools
  • Train and coach team members on maintaining best possible medication lists in Commons and post-discharge medication reconciliation
  • Develop medication-management and disease-state clinical pathways and protocols
  • Work toward collaborative practice agreements enabling pharmacist assessments, laboratory orders, and medication initiation or adjustment per protocol

Requirements

What you’ll need
  • Active, unrestricted pharmacist license from the Massachusetts Board of Registration in Pharmacy at the time of hire, maintained in good standing throughout employment
  • Completion of both a PGY-1 Pharmacy Practice Residency and a PGY-2 Ambulatory Care Pharmacy Residency, or at least 2 years of experience as a clinical pharmacist in an ambulatory care setting
  • Completion of Cityblock’s medical clearance requirements, including evidence of immunity to MMR, Hepatitis B, Varicella, and a TB screen, unless an approved medical or religious accommodation applies
