About the role
Key responsibilities & impact- Provide exceptional neurological rehabilitation for patients with traumatic brain injuries, spinal cord injuries, CVAs, and other neurological or complex orthopedic diagnoses in their homes and communities
- Assess, counsel, and direct medically complex patients
- Develop individualized treatment plans addressing adjustment to disability, resource linkage, and behavior modification
- Collaborate within interdisciplinary teams to create integrated treatment plans practiced in patients' homes and communities
- Provide patient-centered, one-on-one intensive individualized services
Requirements
What you’ll need- Completion of Master's degree in Psychology, Counseling or Social Work (LCSW) and state licensure required
- Excellent planning, organization, writing and time management skills are required
- Ability to communicate with families, professionals, and co-workers is a must
