About the role

Key responsibilities & impact
  • Assess, counsel, and direct medically complex patients
  • Develop individualized treatment plans addressing adjustment to disability, resource linkage, and behavior modification
  • Provide patient-centered neurological rehabilitation in patients' homes and communities
  • Collaborate with interdisciplinary teams to create integrated treatment plans
  • Support patients with traumatic brain injuries, spinal cord injuries, CVAs, and other neurological or complex orthopedic diagnoses

Requirements

What you’ll need
  • Experienced Licensed Clinical Social Worker
  • Completion of a Master's degree in Social Work is required
  • State licensure as an LCSW is required
  • Excellent planning, organization, writing, and time management skills are required
  • Ability to communicate with families, professionals, and co-workers
  • Must accept patients for the duration of their stay for PRN positions
