About the role
Key responsibilities & impact- Assess, counsel, and direct medically complex patients
- Develop individualized treatment plans
- Assist patients with adjustment to disability
- Link patients with appropriate resources
- Address behavior modification
- Provide patient-centered care in patients' homes and communities
- Collaborate with interdisciplinary teams to create integrated treatment plans
- Provide intensive, individualized services for patients recovering from neurological injuries
Requirements
What you’ll need- Completion of a Master's degree in Social Work is required
- State licensure as an LCSW is required
- Excellent planning, organization, writing, and time management skills are required
- Ability to communicate with families, professionals, and co-workers
- Experience working with traumatic brain injuries, spinal cord injuries, CVAs, neurological, or complex orthopedic diagnoses is relevant to the role
- Ability to provide care in patients' homes and communities
- Must accept patients for the duration of their stay for PRN positions
