About the role
Key responsibilities & impact- Assess, counsel, and direct medically complex patients
- Develop individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
- Provide patient-centered neurological rehabilitation in patients' homes and communities
- Accept patients for the duration of their stay in PRN assignments
- Collaborate with interdisciplinary teams to create integrated treatment plans
- Deliver intensive, individualized one-on-one services in home and community settings
Requirements
What you’ll need- Completion of a Master's degree in Social Work
- State licensure as an LCSW
- Excellent planning, organization, writing, and time management skills
- Ability to communicate with families, professionals, and co-workers
- Experience working with traumatic brain injuries, spinal cord injuries, CVAs, and other neurological or complex orthopedic diagnoses is indicated by the role context
