About the role
Key responsibilities & impact- Assess, counsel, and direct medically complex patients
- Develop individualized treatment plans addressing adjustment to disability, resource linkage, and behavior modification
- Provide intensive, individualized, one-on-one services in patients' homes and communities
- Collaborate with interdisciplinary teams to create integrated treatment plans
- Provide care for traumatic brain injuries, spinal cord injuries, CVAs, and other neurological or complex orthopedic diagnoses
Requirements
What you’ll need- Completion of a Master's degree in Social Work
- State licensure as an LCSW
- Excellent planning, organization, writing, and time management skills
- Ability to communicate with families, professionals, and co-workers
- Experience providing care to medically complex and neurological rehabilitation patients is indicated
