About the role
Key responsibilities & impact- Assess medically complex patients
- Counsel medically complex patients
- Direct medically complex patients
- Develop individualized treatment plans
- Assist with adjustment to disability
- Provide resource linkage
- Support behavior modification
- Work with interdisciplinary teams to create integrated treatment plans
- Deliver one-on-one treatment in patients' homes and communities
- Provide patient-centered neurological rehabilitation for traumatic brain injuries, spinal cord injuries, CVAs, and complex orthopedic diagnoses
Requirements
What you’ll need- Completion of a Master's degree in Social Work
- State licensure as an LCSW
- Experienced Licensed Clinical Social Worker
- Excellent planning, organization, writing, and time management skills
- Ability to communicate with families, professionals, and co-workers
- Ability to provide care in Spring Valley, CA and surrounding areas
- PRN clinicians must accept patients for the duration of their stay
