About the role

Key responsibilities & impact
  • Participate in planning and coordination of total patient care with the RN and physician’s plan of care
  • Follow the nursing care plan for each patient
  • Accurately report and record the patient’s condition and care, including signs and symptoms indicating change
  • Prepare clinical/progress notes and summaries
  • Assist patients with activities of daily living while teaching self-care techniques
  • Provide and maintain a safe patient environment
  • Assist the physician and Registered Nurse in complex nursing situations
  • Follow universal precautions during patient care
  • Follow emergency procedures during catastrophic events, accidents, injuries, or significant changes in patient condition
  • Provide patient-centered neurological rehabilitation in patients’ homes and communities

Requirements

What you’ll need
  • 1 year experience as an LPN in a home care setting
  • Current state license as an LPN
  • Graduate of an accredited program of practical nursing
  • Proof of current CPR certification
  • Proof of current negative TB test
  • Ability to work a flexible schedule
  • Excellent interpersonal skills
  • Valid driver’s license and reliable transportation
  • Ability to work a flexible schedule and travel locally
