About the role
Key responsibilities & impact- Provide neurological rehabilitation care in patients' homes and communities
- Participate in planning and coordinating total patient care with the RN and physician's plan of care
- Follow the nursing care plan for each patient
- Accurately report and record patient condition and care, including signs and symptoms indicating change
- Prepare clinical/progress notes and summaries
- Assist patients with activities of daily living and teach self-care techniques
- Provide and maintain a safe patient environment
- Assist the physician and Registered Nurse in complex nursing situations
- Follow universal precautions during patient care
- Follow emergency procedures during catastrophic events, accidents, injuries, or significant changes in patient condition
- Collaborate with interdisciplinary teams developing integrated treatment plans
Requirements
What you’ll need- 1 year experience as an LPN in a home care setting
- Current state license as an LPN
- Graduate of an accredited program of practical nursing
- Proof of current CPR certification
- Proof of current negative TB test
- Ability to work flexible schedule
- Excellent interpersonal skills
- Valid driver's license and reliable transportation
- Ability to work a flexible schedule and travel locally
