About the role

Key responsibilities & impact
  • Visit patients in their homes for one-on-one visits
  • Complete initial and ongoing assessments to identify physical, psychosocial, and environmental needs of home health patients
  • Assess patients’ conditions and chart observations
  • Perform evaluation tasks, including vital signs and medication review
  • Administer medication as prescribed by the patient’s physician
  • Educate patients and families on proper home health care strategies and procedures
  • Recommend devices or tools that may improve patients’ quality of life
  • Communicate effectively with patients, family members, team members, and other healthcare professionals
  • Participate in integrated treatment plans through interdisciplinary teams
  • Provide neurological rehabilitation for traumatic brain injuries, spinal cord injuries, CVAs, and complex orthopedic diagnoses in patients’ homes and communities

Requirements

What you’ll need
  • Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing
  • Current state license as an RN
  • Proof of current CPR certification
  • Compassionate, friendly demeanor
  • Excellent communication skills
  • Current driver’s license and reliable transportation
  • Ability to work a flexible schedule and travel locally
  • Experienced RN preferred by the description
  • Must provide patient care in Southlake, Texas and surrounding areas
