About the role

Key responsibilities & impact
  • Conduct continual telephonic outreach to identified and referred members to engage them with care management services
  • Receive and manage referrals from data reports, clinical care management team members, PCPs, and other service providers
  • Complete and document member screening tools
  • Refer clinical needs to the appropriate clinical care management team member for assessment and follow-up
  • Schedule members for face-to-face or telephonic care management follow-up encounters
  • Meet daily and monthly program productivity and engagement metrics
  • Complete assignments within expected timeframes
  • Notify the supervisor of referral, assignment, or duty-related issues
  • Perform duties that keep operations organized and functional
  • Attend departmental and corporate meetings
  • Uphold CCNC goals, objectives, standards, department guidelines, company policies, and HIPAA regulations
  • Work with members, providers, and staff to implement program interventions, document activities, and refer appropriate members to professional staff

Requirements

What you’ll need
  • High School/GED
  • Minimum of two years previous work experience, preferably in a health care setting
  • Prior telephonic call center experience desired
  • Healthcare experience and medical terminology knowledge highly preferred
  • Bilingual preferred
  • Strong oral and written communication proficiency
