About the role

Key responsibilities & impact
  • Serve as a liaison between health and social services and the community
  • Facilitate access to services and improve the quality and cultural competence of service delivery
  • Increase health knowledge and self-sufficiency through outreach, community education, informal counseling, social support, and advocacy
  • Engage prospective care management program participants identified through data analysis and referral sources
  • Provide community outreach services, including home visits
  • Assist individuals with accessing transportation services
  • Educate enrollees on healthy behaviors and community resources
  • Provide telephonic and face-to-face follow-up with clients and service planning partners
  • Coordinate reminder calls, medication, and medical appointments
  • Assist care managers in securing and identifying referrals to medical, behavioral health, and social assistance providers
  • Provide oral and written status updates regarding client alerts, progress, and needs
  • Maintain case documentation according to policies and procedures
  • Provide activities promoting socialization, recovery, self-advocacy, natural supports, and community living skills
  • Increase enrollees’ knowledge of health conditions and improve adherence to prescribed treatment
  • Travel throughout Pierce County and work in the field at least 50% of the time, with field visit appointments up to 75% of the time
  • Perform other duties as assigned

Requirements

What you’ll need
  • Three (3) years of job-related experience providing medical, mental health or substance abuse-focused services to individuals with chronic medical conditions and/or severe and persistent mental illness
  • Comfortable working with at-risk populations and communities of color
  • Experience sourcing local resources for persons with behavioral and/or physical health needs
  • Experience using a health plan care management system or electronic medical record system
  • High school diploma or equivalent
  • Certification as a Peer Specialist if lacking minimum experience and education requirements
  • Special skill sets such as speaking another language or certification as a Community Health Worker may be applicable
  • Valid state-issued driver’s license
  • Acceptable driving record
  • Reliable transportation
  • Successfully pass a criminal background check
  • Must not be sanctioned or excluded from participation in federal or state healthcare programs
  • Meet the vaccination requirement, subject to medical or religious exemptions
  • Excellent verbal and written communication skills
  • Excellent problem-solving skills
  • Proficient in Microsoft Word, Excel, and Outlook
  • Ability to drive
  • Ability to collaborate with team members
  • Ability to learn and prioritize multiple tasks and handle demanding situations
  • Analytical, problem-solving, and critical-thinking ability
