Department of Corrections Case Manager
Community Medical Services. Serve as the primary contact for behavioral health services outside the counseling arena under the direct supervision of the Clinic Manager .
About the role
Key responsibilities & impact- Serve as the primary contact for behavioral health services outside the counseling arena under the direct supervision of the Clinic Manager
- Conduct new client intake and facilitate treatment program orientation
- Complete wellness-check phone calls, progress notes, clinical documentation, and required service documentation
- Coordinate internal support needs to reduce treatment barriers
- Screen potential clients and schedule intake appointments
- Assist with intake documentation, releases of information, care coordination letters, fee agreements, advanced directives, and language assistance forms
- Monitor Medicaid and other insurance paperwork, completing required intake and discharge forms
- Coordinate care with primary care providers and other mutual providers, including mental health treatment, DCS, and probation/parole
- Arrange transportation as needed
- Facilitate internal transfers between CMS locations and transfers to external MAT programs
- Arrange courtesy dosing and request exceptions for take-home medication policy
- Collaborate with counselors regarding AWOLs and abnormal drug screens
- Facilitate and track the Substance Use Disorder Voucher process and communicate with clients
- Work two days per week at the Columbus Broad St. location
Requirements
What you’ll need- Must be 21 years of age
- High school diploma or GED plus 2 years of qualifying education, behavioral health experience, or a combination of both
- Associate degree plus 2 years full-time behavioral health work experience
- Bachelor's degree plus 1 year full-time behavioral health work experience
- Master's or Bachelor's in a behavioral health-related field
- Valid state-issued driver's license
- 39-month MVR clearance required
- Experience working with multi-cultural and low socio-economic individuals
- Experience in an outpatient setting
- Residential experience and knowledge of psychotropic medications
- Knowledge of behavioral management, de-escalation methods, crisis intervention, and defusing techniques
- Ability to apply crisis and de-escalation techniques to clients at various levels of crisis
- Academic and working knowledge of DSM-5, community resources, clinical methods and procedures, and their application
