About the role
Key responsibilities & impact- Conduct virtual patient visits and clinical assessments
- Perform triage and identify early warning signs or risk trends
- Support chronic disease management and patient education
- Monitor telehealth data and escalate concerns appropriately
- Help prevent avoidable ER visits and rehospitalizations
- Collaborate with interdisciplinary care teams
- Document patient interactions accurately and timely
- Participate in care planning and performance improvement initiatives
- Support development of virtual care best practices
Requirements
What you’ll need- Active, unencumbered Florida LPN license
- Minimum of 2 years of home health experience
- Experience with triage and hospitalization prevention
- Strong clinical judgment and communication skills
- Comfortable using telehealth platforms and EMR systems
- High-speed internet connection
- HIPAA-compliant, distraction-free workplace
- Experience working with high-risk or medically complex patients preferred
- Knowledge of chronic disease management preferred
- Computer savvy, with ability to use Teams, Outlook, and Excel
- Employment contingent upon successful completion of an AHCA Level 2 background screening
- Must reside within the state of Florida due to operational, licensing, and/or business requirements
