About the role

Key responsibilities & impact

Conduct virtual patient visits and clinical assessments
Perform triage and identify early warning signs or risk trends
Support chronic disease management and patient education
Monitor telehealth data and escalate concerns appropriately
Help prevent avoidable ER visits and rehospitalizations
Collaborate with interdisciplinary care teams
Document patient interactions accurately and timely
Participate in care planning and performance improvement initiatives
Support development of virtual care best practices

Requirements

What you’ll need

Active, unencumbered Florida LPN license
Minimum of 2 years of home health experience
Experience with triage and hospitalization prevention
Strong clinical judgment and communication skills
Comfortable using telehealth platforms and EMR systems
Reliable internet connection and private remote workspace
Experience working with high-risk or medically complex patients preferred
Knowledge of chronic disease management preferred
Microsoft Office proficiency preferred
Employment is contingent upon successful completion of an AHCA Level 2 background screening
Candidates must reside within the state of Florida due to operational, licensing, and/or business requirements
