About the role

Key responsibilities & impact

Conduct virtual patient visits and clinical assessments
Perform triage and identify early warning signs or risk trends
Support chronic disease management and patient education
Monitor telehealth data and escalate concerns appropriately
Help prevent avoidable ER visits and rehospitalizations
Collaborate with interdisciplinary care teams
Document patient interactions accurately and timely
Participate in care planning and performance improvement initiatives
Support development of virtual care best practices
Provide proactive remote support and clinical intervention to reduce hospitalizations and improve patient outcomes

Requirements

What you’ll need

Active, unencumbered Florida LPN license
Minimum of 2 years of home health experience
Experience with triage and hospitalization prevention
Strong clinical judgment and communication skills
Comfortable using telehealth platforms and EMR systems
High speed internet connection
HIPPA compliant/distraction free workplace
Computer savvy, with ability to use TEAMS, outlook, excel
Experience working with high-risk or medically complex patients (preferred)
Knowledge of chronic disease management (preferred)
Microsoft Office proficiency (preferred)
Successful completion of an AHCA Level 2 background screening
