About the role
Key responsibilities & impact- Conduct in-home assessments for applicants and participants receiving Long-Term Personal Care Services (LT-PCS)
- Evaluate client needs and gather information for eligibility determinations, care planning, and service coordination
- Provide Choice Counseling for Home and Community-Based Services (HCBS) programs when requested
- Educate applicants and family members about Louisiana Medicaid long-term care programs, healthcare services, and community-based support resources
- Develop and maintain person-centered Plans of Care based on individual needs, goals, and functional assessments
- Coordinate services and connect clients with healthcare providers, community organizations, and social service resources
- Complete monitoring visits, client surveys, case documentation, and quality management activities
- Maintain accurate records and ensure adherence to state and program guidelines
- Conduct regular travel throughout Bienville, Bossier, Caddo, Claiborne, DeSoto, Red River, Sabine, and Webster Parishes
Requirements
What you’ll need- Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Public Health, Healthcare Administration, or related field
- Strong verbal and written communication skills
- Interest in working directly with diverse populations
- Louisiana resident
- Reliable transportation
- Valid driver's license and current auto insurance
- Ability to travel daily within the assigned region
- Ability to work independently while managing multiple client cases and priorities
- Preferred: experience in social services, human services, healthcare, case management, care coordination, community outreach, or client advocacy
- Preferred: experience working with older adults, individuals with disabilities, or vulnerable populations
- Preferred: knowledge of Medicaid programs, long-term care services, healthcare systems, or HCBS
- Preferred: experience conducting assessments, interviewing clients, or developing care plans
