About the role
Key responsibilities & impact- Conduct patient intake and initial triage through the inbound digital channel
- Guide patients throughout their care journey by coordinating appointments, referrals, and treatment plans
- Serve as the patient’s point of reference, answering questions and providing guidance on how to use available services
- Promote patient engagement in self-care
- Proactively monitor patients, identifying risks and ensuring continuity of care
- Accurately record and update information in the electronic medical record
- Collaborate with multidisciplinary teams to ensure comprehensive care
- Contribute to the continuous improvement of processes and protocols for the digital hospital
Requirements
What you’ll need- Valid, up-to-date medical disability documentation (position exclusively for persons with disabilities)
- Bachelor’s degree in Nursing and an active COREN registration
- Previous experience in digital health, telemedicine, or remote patient care
- Concierge-oriented profile, with clear communication, empathy, a focus on the patient experience, and problem-solving skills
- Proficiency with digital tools and healthcare systems, including electronic medical records and telehealth platforms
- Strong time-management skills and the ability to handle multiple patient interactions simultaneously
- Availability to work a 12-on/36-off schedule, from 7:00 a.m. to 7:00 p.m.
- Graduate-level qualification in Public Health, Community Health, Family Health, Healthcare Management, or a related field (preferred qualification)
- Experience with patient navigation programs or chronic disease management (preferred qualification)
