About the role

Key responsibilities & impact
  • Develop a framework for building and growing the CookUnity RDN network, including clinical competencies, disease-area specializations, cultural and linguistic attributes, and recruitment and vetting processes
  • Co-develop disease-specific nutrition protocols for diabetes, oncology, weight management with GLP-1s, and cardiovascular conditions
  • Define nutrition assessment frameworks, intake tools, flagging criteria, and escalation pathways
  • Build structured visit frameworks for nutrition education and counseling
  • Integrate Motivational Interviewing, Stages of Change, and SMART goal-setting into care delivery standards
  • Establish culturally responsive practice standards
  • Develop clinical documentation standards, quality metrics, and outcome tracking
  • Build and curate a clinical resource library with patient education materials, counseling tools, and handouts
  • Carry a direct-care caseload of approximately 8–12 active members
  • Conduct comprehensive nutrition assessments and develop individualized care plans incorporating medically tailored meal planning
  • Deliver evidence-based nutrition counseling and education
  • Collaborate with health plan care teams and care coordinators on care management and referral pathways
  • Provide clinical oversight and quality assurance for the RDN network
  • Conduct chart audits and clinical case reviews
  • Facilitate monthly peer review or case consultation sessions
  • Serve as the primary clinical escalation point for complex, high-acuity, or ethically sensitive cases
  • Lead onboarding and training of contracted and subcontracted RDNs
  • Identify training needs and coordinate continuing education or skill-building resources

Requirements

What you’ll need
  • Active Registered Dietitian Nutritionist (RDN) credential in good standing with the Commission on Dietetic Registration (CDR)
  • Active state dietitian licensure where required by state of practice (e.g., CDN in New York)
  • 5+ years of clinical RD experience with a meaningful focus on chronic disease management — diabetes, cardiovascular disease, oncology, weight management, or combinations thereof
  • Demonstrated experience delivering Medical Nutrition Therapy (MNT) and nutrition counseling to vulnerable, medically complex, and/or diverse patient populations
  • Familiarity with behavior change frameworks, including Motivational Interviewing
  • Strong clinical documentation skills and comfort with telehealth or virtual care delivery
  • Self-directed and comfortable building processes in a fast-moving, start-up environment
  • CDCES — Certified Diabetes Care and Education Specialist (strongly preferred; required for certain plan contracts)
  • NBC-HWC — National Board Certified Health and Wellness Coach
  • CSO — Board Certified Specialist in Oncology Nutrition
  • CSOWM — Board Certified Specialist in Obesity and Weight Management (particularly relevant given GLP-1 meal line)
  • Washington State Licensed Dietitian (LD) licensure, or willingness to obtain (preferred for current contract pipeline; licensure support available)
  • FAND — Fellow of the Academy of Nutrition and Dietetics
  • Formal Motivational Interviewing training or certification (MINT network or equivalent)
  • Experience in medically tailored meal (MTM), food-as-medicine, or community-based nutrition programs
  • Prior experience supervising or mentoring RDNs, dietetic interns, or clinical nutrition staff
