About the role
Key responsibilities & impact- Evaluate patients using targeted intervention business rules and processes to identify needed medical services
- Make appropriate referrals to medical services staff and coordinate required services according to the benefit plan
- Communicate health care service delivery based on outcomes/reviews noted by the nurse or medical director
- Perform non-medical research for establishing, maintaining, and closing open cases
- Answer telephone calls from providers and members and take accurate messages
- Support electronic transmission review and referrals as appropriate
- Use internal tools to determine required steps based on clinical requirements and plan guidelines
- Maintain accurate and complete documentation meeting risk management, regulatory, and accreditation requirements
- Communicate internally and externally to enhance medical management services
- Obtain discharge dates and make appropriate referrals to the clinical team for additional follow-up
- Complete work independently while exercising judgment and critical thinking
- Meet quality and quantity performance standards in a rapidly changing environment
Requirements
What you’ll need- Relevant work experience
- Adept at problem solving and decision making skills
- High school diploma
- Preferred: Telephonic experience
- Preferred: Member outreach experience
