About the role

Key responsibilities & impact

Provide approximately 45-minute in-home health evaluations
Engage one-on-one with health plan members and answer health questions
Review medications and medical histories
Perform physical evaluations
Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry
Use an iPad and intuitive clinical workflow for each evaluation
Provide education and help members understand and manage their health
Travel locally and within licensed states as needed
Conduct virtual visits as needed
Provide Signify Health Diagnostic & Preventive Services products
Build trusted relationships and help bridge gaps in care

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Multi-state licenses (preferred)
Able to cover the following primary locations: Otsego County, Albany NY
Able to work safely and comfortably in members’ homes, including environments with household pets, secondhand smoke and potential airborne allergens or irritants
Willingness and ability to travel locally and within licensed states, as needed
Ability to conduct virtual visits as needed
Ability to use an iPad and clinical workflow for evaluations
