About the role
Key responsibilities & impact- Provide in-home health evaluations in members’ homes
- Travel up to 100% of the time across a defined coverage area, potentially requiring overnight hotel stays, car rentals, and/or flights
- Build trusted relationships and help improve health outcomes
- Conduct approximately 45-minute visits
- Review medications and medical history
- Perform physical evaluations
- Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening
- Use an iPad and clinical workflow for each evaluation
- Conduct virtual visits as needed
- Provide Signify Health diagnostic and preventive services
- Visit health plan members nationwide and connect them with appropriate care
- Provide health education and answer members’ questions
- Work in members’ private residences
Requirements
What you’ll need- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within state and flex to other areas of business need
- Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants
- Nurse Practitioner or Physician Assistant qualification
- Availability for 40 hours per week
- Willingness and ability to travel locally and within licensed states
- Ability to conduct virtual visits as needed
