About the role

Key responsibilities & impact

Provide In-Home Health Evaluations in health plan members’ private residences
Build trusted relationships and help improve health outcomes
Conduct approximately 45-minute visits
Answer health questions
Review medications and medical history
Perform physical evaluations
Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry and bone density screening
Use an iPad and clinical workflow for each evaluation
Visit people nationwide and connect them to appropriate care
Conduct virtual visits as needed
Provide Signify Health services, including Diagnostic & Preventive Services products

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s) Dayton, OH
Board certification (required)
Multi-state licenses (preferred)
Ability to work flexibly and travel locally and within licensed states, as needed
Ability to conduct virtual visits as needed
Ability to safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
