About the role

Key responsibilities & impact

Provide approximately 45-minute in-home health evaluations
Engage one-on-one with health plan members and answer health questions
Review medications and medical history
Perform physical evaluations
Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
Use an iPad and intuitive clinical workflow for each evaluation
Visit people nationwide to connect them with appropriate care
Conduct virtual visits as needed
Provide Signify Health services, including Diagnostic & Preventive Services products
Travel locally and within licensed states as needed
Build trusted relationships and help improve health outcomes

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Multi-state licenses (preferred)
Able to cover Las Vegas/Clark County
Able to travel locally and within licensed states as needed
Able to perform duties safely in private residences that may contain household pets, secondhand smoke, and airborne allergens or irritants
Nurse Practitioner or Physician Assistant qualification
