About the role

Key responsibilities & impact

Provide approximately 45-minute in-home health evaluations
Engage with health plan members in their homes and build trusted relationships
Review medications and medical history
Perform physical evaluations
Conduct additional diagnostic tests when ordered, including diabetic eye exams, spirometry, and bone density screening
Use an iPad and clinical workflow for each evaluation
Conduct virtual visits as needed
Provide Signify Health Diagnostic & Preventive Services products
Help identify connections to appropriate care and bridge gaps in care
Visit members nationwide as part of a network of purpose-driven clinicians

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Multi-state licenses (preferred)
Ability to work safely and comfortably in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants
Flexibility to travel locally and within licensed states as needed
Nurse Practitioner or Physician Assistant qualification
