About the role

Key responsibilities & impact

Provide in-home health evaluations in members’ private residences
Conduct approximately 45-minute visits
Review medications and medical history
Perform physical evaluations
Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
Use an iPad and clinical workflow for each evaluation
Engage members, answer health questions, and provide health education
Help identify connections to appropriate care
Provide Signify Health Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening
Conduct virtual visits as needed
Travel locally and within licensed states as needed

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Board certification required
Multi-state licenses preferred
Ability to travel locally and within licensed states as needed
Ability to conduct virtual visits as needed
Ability to safely perform duties in homes containing household pets, secondhand smoke, and other potential airborne allergens or irritants
