About the role
Key responsibilities & impact- Provide in-home health evaluations in members’ residences
- Conduct approximately 45-minute visits
- Review medications and medical history
- Perform physical evaluations
- Perform additional diagnostic tests when ordered by the health plan
- Answer health questions and provide health education
- Identify connections to appropriate care and help bridge gaps in care
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide Signify Health diagnostic and preventive services
Requirements
What you’ll need- Active, unrestricted license(s) in coverage area(s)
- Board certification required
- Multi-state licenses preferred
- Nurse Practitioner or Physician Assistant qualification
- Ability to travel locally and within licensed states as needed
- Ability to conduct visits in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants
- Ability to use an iPad and clinical workflow
- Ability to provide diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screening
