About the role

Key responsibilities & impact

Provide approximately 45-minute in-home health evaluations
Engage with health plan members and build trusted relationships
Review medications and medical histories
Perform physical evaluations
Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
Conduct virtual visits as needed
Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening
Travel locally and within licensed states as needed
Help identify connections to appropriate care and improve health outcomes
Work as part of a national network of purpose-driven clinicians

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Multi-state licenses (preferred)
Ability to travel locally and within licensed states, as needed
Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and other airborne allergens or irritants
Ability to use an iPad and clinical workflow for evaluations
Nurse Practitioner or Physician Assistant qualification
