About the role

Key responsibilities & impact

Provide In-Home Health Evaluations in members’ homes
Conduct approximately 45-minute visits
Review medications and medical history
Perform physical evaluations
Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
Use an iPad and clinical workflow for each evaluation
Conduct virtual visits as needed
Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening
Travel locally and within licensed states as needed
Help identify connections to appropriate care and improve health outcomes

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s) Seattle Snohomish County WA
Board certification (required)
Multi-state licenses (preferred)
Ability to travel locally and within licensed states, as needed
Ability to work safely and comfortably in private residences that may contain household pets, secondhand smoke, and other airborne allergens or irritants
