About the role

Key responsibilities & impact

Provide In-Home Health Evaluations in members’ homes
Engage one-on-one with members and answer health questions
Review medications and medical history
Perform physical evaluations
Provide additional diagnostic tests when ordered, including diabetic eye exams, spirometry and bone density screening
Use an iPad and clinical workflow for each evaluation
Conduct virtual visits as needed
Provide Signify Health Diagnostic & Preventive Services
Travel locally and within licensed states
Build trusted relationships and help identify appropriate care and referrals
Visit people nationwide to support improved health outcomes

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Able to obtain and maintain multi-state licenses in coverage area, as required
Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
Able to travel locally and within licensed states
Able to travel up to 100% of the time across a defined coverage area
May require overnight hotel stays, car rentals and/or flights
MD or DO qualification stated in the required and preferred qualifications
