About the role

Key responsibilities & impact

Provide in-Home Health Evaluations in members’ homes
Conduct approximately 45-minute visits with one-on-one health conversations
Review medications and medical history
Perform physical evaluations
Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
Use an iPad and clinical workflow for each evaluation
Visit people nationwide to connect them with appropriate care
Conduct virtual visits as needed
Provide Signify Health Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening
Travel locally and within licensed states as needed
Do not prescribe medicine, order lab tests, or alter current treatment regimens

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
MD or DO qualification
Ability to travel locally and within licensed states, as needed
Ability to conduct virtual visits as needed
Ability to safely and comfortably perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants
Multi-state licenses preferred
