About the role

Key responsibilities & impact

Provide approximately 45-minute in-home health evaluations
Engage patients one-on-one and answer health questions
Review medications and medical histories
Perform physical evaluations
Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
Use an iPad and clinical workflow for each evaluation
Visit health plan members in their homes to help identify care gaps and connect them with appropriate care
Conduct virtual visits as needed
Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening
Travel locally and within licensed states as needed

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Ability to travel locally and within licensed states, as needed
Ability to conduct virtual visits as needed
Ability to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants
MD or DO qualification implied by the Required & Preferred Qualifications section
Multi-state licenses preferred
