About the role

Key responsibilities & impact

Provide in-home health evaluations in members’ residences
Engage with members and build trusted relationships
Review medications and medical history
Perform physical evaluations
Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
Answer health questions and help bridge gaps in care
Use an iPad and clinical workflow for each evaluation
Provide education and connections to appropriate care
Conduct virtual visits as needed
Provide Signify Health services, including Diagnostic & Preventive Services
Travel across a defined coverage area, potentially requiring overnight hotel stays, car rentals and/or flights
Visit members nationwide as part of a clinician network

Requirements

What you’ll need

Primary residency in Connecticut
Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Able to obtain and maintain multi-state licenses in coverage area, as required
Able to cover all counties within Connecticut and flex to other areas in cases of business need
Able to travel locally and within licensed states, up to 100% of the time
Able to perform duties safely in homes that may contain household pets, secondhand smoke, airborne allergens or irritants
Nurse Practitioner or Physician Assistant qualification
Ability to conduct virtual visits as needed
Ability to provide Diagnostic & Preventive Services, including spirometry and diabetic eye exams
Ability to use an iPad and clinical workflow
40 hours per week
