About the role

Key responsibilities & impact

Provide In-Home Health Evaluations in health plan members’ homes
Engage with people and build trusted relationships to help improve health outcomes
Travel across a defined coverage area, potentially requiring overnight hotel stays, car rentals and/or flights
Conduct approximately 45-minute visits
Review medications and medical history
Perform physical evaluations
Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams, spirometry and bone density screening
Use an iPad and clinical workflow for each evaluation
Conduct virtual visits as needed
Provide Signify Health services, including Diagnostic & Preventive Services products
Provide education and answer health questions without prescribing medicine, ordering lab tests or altering current treatment regimens

Requirements

What you’ll need

Full-time Nurse Practitioner or Physician Assistant
Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Able to obtain and maintain multi-state licenses in coverage area, as required
Able to cover all counties within state and flex to other areas of business need
Able to travel locally and within licensed states, primarily up to 100% of the time
Able to perform duties safely in homes that may contain household pets, secondhand smoke and other airborne allergens or irritants
Able to use an iPad and clinical workflow
Anticipated weekly hours: 40
