About the role

Key responsibilities & impact

Provide in-home health evaluations in members’ residences
Travel across a defined coverage area, potentially requiring overnight hotel stays, car rentals and/or flights
Conduct approximately 45-minute visits with one-on-one health discussions
Review medications and medical history
Perform physical evaluations
Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry and bone density screening
Use an iPad and clinical workflow for each evaluation
Conduct virtual visits as needed
Provide Signify Health Diagnostic & Preventive Services
Build trusted relationships and help improve health outcomes
Make referrals to members’ health plans and encourage involvement of loved ones when appropriate
Ensure primary care physicians are aware of members’ health and quality of life

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Able to obtain and maintain multi-state licenses in coverage area, as required
MD or DO qualification
Able to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, airborne allergens or irritants
Willingness and ability to travel locally and within licensed states, up to 100% of the time
Ability to conduct virtual visits as needed
