About the role

Key responsibilities & impact

Provide in-home health evaluations for Signify Health members
Conduct approximately 45-minute visits, including medication and medical history reviews and physical evaluations
Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams and spirometry
Use an iPad and clinical workflow during each evaluation
Answer health questions and help bridge gaps in care
Build trusted relationships and help improve health outcomes
Travel locally and within licensed states as needed
Conduct virtual visits as needed
Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening
Work in members’ private residences

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Multi-state licenses (preferred)
Able to cover the primary location: Penobscot, ME
Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants
Nurse Practitioner or Physician Assistant qualification
