About the role

Key responsibilities & impact

Provide approximately 45-minute In-Home Health Evaluations
Engage with health plan members in their homes and build trusted relationships
Review medications and medical history
Perform physical evaluations
Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
Use an iPad and clinical workflow for each evaluation
Visit people nationwide to connect them with appropriate care
Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening
Conduct virtual visits as needed
Travel locally and within licensed states as needed
Work as part of a national network of purpose-driven clinicians

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Multi-state licenses (preferred)
Able to cover Manhattan and surrounding area
Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants
Nurse Practitioner or Physician Assistant qualification
Ability to travel locally and within licensed states as needed
Ability to conduct virtual visits as needed
