About the role

Key responsibilities & impact

Provide telephonic patient care from a home-based setting
Collaborate with health care professionals to provide meaningful patient experiences
Develop, implement, and evaluate comprehensive plans of care for patients with multiple disease states
Make outbound calls to provide education and support for new medications
Support continuity of care for defined patient populations through telecommunications nursing outreach programs
Educate patients on medication adherence and disease-state training
Use critical thinking and clinical expertise in patient interactions
Work Monday through Friday from 10:30 a.m. to 7:00 p.m. CST
Work a set schedule in a full-time hourly position

Requirements

What you’ll need

Must hold an unrestricted Registered Nurse license in the state of residence
Must have multi-state/compact privileges and ability to be licensed in all non-compact states, territories and the District of Columbia based on business needs
Must meet additional requirements imposed by applicable nursing boards
Must be based in Texas or Arizona
3+ years of clinical RN experience
Experience using Microsoft Office, including Word, Excel and Outlook
RN Diploma, Associate’s or Bachelor’s in Nursing
Excellent written and verbal customer service skills
Advanced computer skills
Bachelor’s degree preferred
Previous telephonic nursing experience preferred
EPIC systems experience preferred
Licensure in multiple states preferred
